If you receive care at our facility and are a having difficulty
creating or registering a
MyHealtheVet
account, one-on-one assistance is available. Please contact
our Medical Library Team at (602) 222-6411, Monday through Friday
(excluding federal holidays), 8:00 a.m. to 4:30 p.m. Outside
of regular hours, call and leave a message giving your name and
phone number, and we will contact you the next business day.

Patient Advocates
If you have a compliment, complaint, or other issue which you
believe requires resolution, please contact one of our patient advocates.
Sarah Nelson, Lead Patient Advocate, or Rex Patterson or Karen

on the first floor of Building 1, Room 1147. Their fax number

also available to assist you with concerns you may have as an inpatient
or outpatient.

manner. Your questions, concerns and suggestions for improved care
