 Enrollment Basics

Enrollment can be completed in several different ways:

It is advised that you have your DD-214, social security number, and current contact information available when applying for eligibility.  Once your application is received and processed, you will receive written notification of your enrollment status in the VA health care system. If you have not received this written notification of enrollment status within 14 days after submitting your enrollment information, please contact the Phoenix Eligibility & Enrollment Office at 602-222-6508 to follow-up on your enrollment application

Special Eligibility and Coordination of Care for Combat Veterans Serving in Combat Theater After 11/11/1998-Returning Service members (OEF/OIF/OND)
Veterans who served in a theater of combat operations also have special eligibility for VA health care. Under the "Combat Veteran" authority, VA provides cost-free health care services and nursing home care for conditions possibly related to the Veterans combat service and enrollment in Priority Group 6 or higher for 5 years from the date of discharge or release from active duty, unless eligible for enrollment in a higher priority group.

Combat Veterans who enroll with VA under this enhanced Combat Veteran authority will continue to be enrolled even after their enhanced eligibility period ends, although they may be shifted to a different Priority Group, depending on their income level, and required to make applicable copays. Additionally, for care not related to combat service, copays may be required depending on their financial assessment and other special eligibility factors.
NOTE: The 5-year enrollment period applicable to these Veterans begins on the discharge or separation date of the service member from active duty military service, or in the case of multiple call-ups, the most recent discharge date.

OEF/OIF/OND Combat Veterans Services
The Transition and Care Management Program holds a general orientation meeting for all OEF/OIF/OND Veterans from 2:00pm-3:00pm on the 2nd and 4th Thursday of each month at the Phoenix VA and the 1st and 3rd Tuesday of each month at the Southeast CBOC. Topics covered include common post-deployment concerns, overview of the Department of Veterans Affairs, and discussion of available resources to meet OEF/OIF/OND Veteran needs. You do not need to be enrolled at the Phoenix VAMC in order to attend this orientation.
Veterans can complete a “Patient Request for Social Work Services” via telephone or in person at the Amethyst Clinic to request assistance on topics not covered here or during orientation. The requests will be reviewed and addressed within the next two to seven business days depending on urgency. Please note that this process is not meant for emergency assistance.

Women Veterans Services & Self-Referrals
The Phoenix VA offers gender-specific specialty care and comprehensive medical and mental health services for women Veterans. If you have any questions regarding the Women Veterans Health Program, please call the medical center at (602) 277-5551 (toll-free 1-800-554-7174), then dial ext. 1-2272 or 1-6764 to reach Kristen Nordquist, LCSW, Women Veterans Program Manager. Mental Health Services & Self-Referrals
After completing enrollment into the Phoenix VA Health Care System, Veterans can make a self-referral to Phoenix VA mental health services within the Jade-Opal Clinic by completing a walk-in Monday-Friday 0730-1600 or calling 602-222-2752 to schedule an appointment.
If a mental health crisis occurs outside of regular VA business hours (to include weekends and holidays), Veterans can present to the Phoenix VA Emergency Department which is open 24 hours a day. The Crisis Line is also available 24 hours a day at 1-800-273-TALK, press 1 for Veteran’s Crisis Line. Anytime you are at risk for injuring yourself or others, please call “911” to access emergency services immediately.

Military Sexual Trauma (MST) Services
Veterans who experienced Military Sexual Trauma (MST) may be eligible to receive free MST-related care, even if they are not eligible for other VA care. It is not required to have documentation, or that the incident was reported when it happened to receive counseling services. Please contact the MST Coordinator at 602-277-5551 ext. 1-4900, for more information or to engage in services.

Homeless Veteran Services
The Phoenix VA offers a “one-stop shop” for Homeless Veteran Services at the Community Resource & Referral Center (CRRC). These services are available Monday-Friday 0730-1630 at 1500 E. Thomas Rd, Suite 106 Phoenix, AZ 85014. Services include: eligibility verification & registration, limited clothing and hygiene items, assessment for healthcare and housing needs, peer support services, employment services, medical care, substance abuse services, assistance with benefits and claims, connection to emergency shelter, and assessment and screenings for permanent housing. Shuttle services available from Phoenix VA to CRRC. Additionally available is the National Call Center for Homeless Veterans, available 24/7, at 1-877-424-3838.                                   

Accessing the Veteran Justice Outreach Initiative (VJO) Program
The aim of the Veteran’s Justice Outreach (VJO) Program is to avoid the unnecessary criminalization of mental illness and extended incarceration among Veterans by ensuring that eligible, justice-involved Veterans have timely access to the Veterans Health Administration (VHA) services, as clinically indicated.
Veterans and families can contact VJO social workers for services Monday through Friday 7:30am – 4:30pm (excluding holidays).

Emergency Department (ED)
The Phoenix VA Emergency Department provides care for any urgent needs, medical or mental health, 24 hours a day, seven days a week. If you are having any life threatening emergencies, call 911 immediately.

If you have a change in health condition and cannot wait for your next appointment with your provider, please contact our Registered Nurses at 602-222-6550 for advice. They are available 24 hours a day, seven days a week.
