Phoenix VA Health Care System
Welcome to the VA from the Research Service! We are excited to be able to work with you all. The purpose of VA research is to improve Veteran’s health care.
In order to make these improvements, your willingness to participate in research is critical, and we thank you in advance for your participation! All research is voluntary and will not affect your VA benefits in any way.
Each study has different qualifications as to who can be enrolled, but everyone can participate in the Million Veteran Program, or MVP. You can find out more about MVP and participate by calling Providencia Morales at 602-277-5551, ext. 2450.
Permission to be Contacted form. This form allows you to select the type of research study you would like to participate in. It also gives us consent to contact you and provide more information about our ongoing studies. You can submit the form by mail, fax (602-212-2047) or in person. The Research Service is located on the campus of the Carl T. Hayden VA Medical Center in building 21, room 116.
