The Southwest Community Based Outpatient Clinic was designed to provide ease of access to care for Veterans located in Avondale, Buckeye, Goodyear, Laveen, Tolleson, West Phoenix and other Southwest Valley communities.

This VA clinic provides Primary Care and Mental Health services. This location also offers Social Work, Nutrition, Clinical Pharmacy Consultation and a Laboratory Draw Station. Although the clinic does not have a pharmacy on site, prescriptions can be picked up at the Outpatient Pharmacy in the main Phoenix VA hospital or can be filled by mail.

Appointments

Veterans are seen by appointment only. Your health care provider will schedule appointments as needed. If you are ill or have questions about your care, you can also call (602) 277-5551. Please call to cancel appointments you cannot keep. This allows the staff to schedule other patients who need to receive care. To make appointments more productive, please arrive 30 minutes before the appointment, bring all medications, home blood pressure records, diabetes diary, outside medical records (including those from other VA facilities) and any other pertinent medical information. The facility has a large parking lot for Veterans and staff.
