The Southwest Community Based Outpatient Clinic was designed to ease access to care for Veterans in Avondale, Buckeye, Goodyear, Laveen, Tolleson, West Phoenix and other Southwest Valley communities.

This VA clinic provides Primary Care and Mental Health services. This location also offers Social Work, Nutrition, Clinical Pharmacy Consultation and a Laboratory Draw Station. Although the clinic does not have a pharmacy on site, prescriptions can be picked up at the Outpatient Pharmacy in the main Phoenix VA hospital or can be filled by mail.


Veterans are seen by appointment only. Your health care provider will schedule appointments as needed. If you are ill or have questions about your care, you can also call 602-222-6550. Please call to cancel appointments you cannot keep, as that allows us to schedule other Veterans who need care.

To make appointments more productive, please arrive 30 minutes before the appointment, and bring:
- All medications
- Home blood pressure records
- Diabetes diary
- Outside medical records (including those from other VA facilities)
- Any other pertinent medical information.


From Loop 101:
- Exit Thomas Road and turn east.
- Turn north on 93rd Avenue.
- The clinic will be on the right.

From Interstate 10:
- Exit at 91st Avenue and turn north
- Proceed approximately 1.5 miles to Thomas Road.
- Turn left (west) on Thomas Road
- Turn north on 93rd Avenue.
- The clinic will be on the right.
