The Midtown VA Health Care Clinic is an outpatient clinic designed to provide ease of access care to Veterans located in the heart of Phoenix. This VA clinic provides primary care and some mental health services, with the exception of services provided in the Jade/Opal Clinics, which remains in the Carl T. Hayden VA Medical Center. However, the entire Post Traumatic Stress Disorder (PTSD) clinic is located in the Midtown Clinic. Other services offered at this location include social work, nutrition, clinical pharmacy consultation and a labratory draw station. Although the clinic does not have a pharmacy on site, prescriptions can be picked up at the outpatient pharmacy in the main Phoenix VA medical center or can be filled by mail.


Veterans are seen on a walk-in basis or by appointment. If you are ill or have questions about your care, you can also call 602-633-6900 or 602-277-5551.

Please call to cancel appointments you cannot keep. This allows the staff to schedule other patients who need to receive care. To make appointments more productive, please arrive 30 minutes before the appointment, bring all medications, home blood pressure records, diabetes diary, outside medical records (including those from other VA facilities) and any other pertinent medical information. Parking is free on the premises, and shuttle services are provided to and from the Carl T. Hayden VA Medical Center on weekdays only.
