Admissions to Community Hospitals
- Anyone wanting to report an ER visit or admit to a community hospital should call the numbers below based on time and date of the week.

Military Sexual Trauma (MST)

MyHealtheVet - Local Help / Authentication
If you receive care at our facility and are a having difficulty creating or registering a MyHealtheVet account, one-on-one assistance is available in room 1109. Please contact your MyHealtheVet Coordinator Victoria Reyes between the hours of 8 a.m. to 4 p.m, Monday through Friday (excluding federal holidays). Outside of regular hours, call and leave a message giving your name and phone number and we will contact you the next business day.

The Carl T. Hayden VA Medical Center has a team of highly-skilled patient representatives who are eager to assist if you have a compliment, complaint, or issues with which you need assistance. The patient representatives:
- serve as liaisons between patients and the medical center and act on the patient's behalf.
- help patients understand their rights and responsibilities.

- Our Release of Information office is who you will need to contact to get information from your medical record. It is located in Room 104E on the first floor of Building 1 at the medical center, near the Emergency Room.
