Patient Safety is the number one priority of the Phoenix VA Health Care System and our friendly staff every day that we deliver your healthcare.  Our providers understand that getting your care in a safe and friendly environment is the most important thing to you and your loved ones.  At Phoenix VA Health Care System, we continuously train our providers in safe procedures for managing your care.

Our facility provides a blanket of protection for patients while under our care through the use of unlimited strategies and tactics that help prevent mistakes.  Here are some of the key elements we use:

•         Two patient identifiers

•         Medication reconciliation

•         Clinical reminders and alarm systems

•         Infection control surveillance to reduce the risk of health care-associated infections

•         Fall mitigation

•         Universal protocols in the operating room to identify safety risks

If you would like to know more about Patient Safety, you can view award winning material from the Joint Commission’s Speak Up Program at the following link: http://www.jointcommission.org/multimedia/ask-your-advocate-to-speak-up/.
