     The Phoenix VA Health Care System's main facility, the Carl T. Hayden VA Medical Center, is located at 650 E. Indian School Road in Phoenix and serves veterans in central Arizona including the rapidly expanding metropolitan Phoenix area.  The medical center provides acute medical, surgical and psychiatric inpatient care, as well as rehabilitation medicine, neurological, and intermediate care.           

     Community Living Center care is available to eligible beneficiaries within the medical center's units or through contractual arrangements with community facilities.  Also, the Ambulatory Care Section offers a full range of services to veteran outpatients.            

     The quality of care provided is assured through multiple review processes designed to enhance patient care through all aspects and correct any identified deficiencies.
