Our Northwest Veterans Affairs Health Care Clinic serves our veterans from El Mirage, Glendale, Peoria, Sun City, Sun City West, Surprise, Wickenburg and Wittman, Arizona.
The clinic is based on a primary care model of health care that has physicians and nurse practitioners. The clinic has its own pharmacy and laboratory and has a full-time social worker, psychologist and dietitian on their staff. There is also a part-time psychiatrist, urologist, cardiologist and gastroenterologist, dermatologist, and geriatrician.

Medical visits are by appointment only.
Your health care provider will coordinate the care you receive from other professionals on the team. If you are ill or have questions about your care, you are encouraged to call (602) 222-2630, or (602) 277-5551, extension 2631.
Please call to cancel any appointments you cannot keep. This allows us to schedule other patients who may be ill.
To make your appointment more productive, we suggest that you arrive 30 minutes before your appointment and bring with you:
- All medications
- Home blood pressure records
- Diabetes diaries
- All outside medical records including those from other VA facilities
- Any other information that may apply to your care.

From the 303 Loop
Exit at Bell Rd, and head east. Turn onto Grand Ave. heading south. Turn right onto N. 101st Ave. End at 10147 Grand Ave.
From the west 101 Loop
Take the Grand Ave. / US 60 Exit [Exit 11] and head north. Turn left onto N. 101st Ave. End at 10147 Grand Ave.
